birth certificate?

Copies of birth certificates can be issued by the HIM office. However, this certificate is not the same as the 'Certificate of Live Birth' which is required for travel or official use. This certificate can be obtained from the Vital Statistics office of the state where the child was born.

If you have questions regarding Admission to Harney District Hospital, getting copies of your Medical Records from the Health Information Management Department or the billing process, please call 541.573.7281 and ask for the appropriate department.

01admissions and discharges:
Harney District Hospital always keeps your information on file, but be prepared to update that information, especially if you have moved, changed insurance or changed jobs since your last visit. Always bring your most recent insurance card when being admitted to the hospital.

Complete and accurate information allows proper billing of your insurance company. Please contact the Admissions Department if you have any questions at (541) 573-7281.

Did you get a bill you don't understand or believe the charges may be wrong? Please contact the Billing Department today. Their staff is ready to assist you with any questions you may have and will ensure your hospital charges are taken care of in an accurate, efficient manner.

01health information management (HIM):

Health Information Services is happy to offer assistance with any medical record needs. Our department is responsible for all emergency room and inpatient hospital service records. Your medical records are covered by HIPAA privacy regulations and our own hospital confidentiality standards.

Harney District Hospital provides a financial assistance program for its patients. This program may lend assistance in the form of adjustments or payment plans to help patients pay for hospital services.
